This case involves an electrician who died from an accident at an amusement park in Wisconsin. The electrician was called in early in the morning to fix a maintenance issue on an indoor roller coaster, but the proper lockout/tagout procedure was not completed. Subsequently, while working on the roller coaster, the ride was somehow activated. The roller coaster train was released from the launch track and came barreling down at the electrician. He was able to survive, but his legs were crushed in the accident and had to be amputated.

I have over 40 years of construction management experience in building construction and 5 years teaching construction management and safety. I have a BS in Civil Engineering, Â MS in Civil Engineering and a JD. I am currently under contract with OSHA providing electrical safety training to local contractors. In total, I have worked over 40 years as a Construction Engineer and Project Manager on building project construction sites, and I now teach construction project management at a university. This accident could have been avoided using standard OSHA lockout/tagout procedure.
